Who we help
We support adults who:
- are medically ready for discharge from hospital, but who require a further period of rehabilitation and recovery
- need a wide range of care and support, including nursing and dementia support
What we do
We provide short term residential and nursing support to adults which may include specialist support, therapy, nursing intervention or a review of overall care needs.
The service is delivered in partnership with Leeds Community Healthcare NHS Trust, bringing together a variety of health and social care specialists to provide a strength based and recovery focussed approach to supporting people.
Our hubs
We have three hubs in Leeds -
East (Seacroft),
North West (Yeadon) and
South (Beeston).
Our hubs work with people to facilitate faster recovery from illness, early hospital discharge and to avoid premature admission to long-term residential care. Offering the right support at the right time, they also provide an alternative to hospital admission.
The Willows
The Willows is a specialist service based within the recovery hub at
South Leeds. The service, provides dedicated, expert support to people who experience psychological and behavioural symptoms of Dementia.
