IMPACT is a service providing follow-on support to those who have found somewhere to live and settled in with help from the
mental health homeless team (MHHT).
Who we help
The IMPACT team support people who are faced with homelessness, have mental health problems and have been supported by the MHHT or HHIB team.
What we do
We help people who are in their own accommodation to sustain their tenancies and settle into the community through activities. We also signpost to education and employment and arrange longer-term housing support if needed.
Eligibility
You may be eligible for ongoing support if you have completed support from the MHHT or HHIB team, or this support is coming to an end.
We plan the type of support you would like, which inlcudes help with:
- being settled and established in your home and community
- accessing healthcare
- building resilience and self-management skills
- linking to other support services
- support and advice with mental health, budgeting advice and support to help apply for benefits and grants
- accessing education, day opportunities and volunteering opportunities
- budgeting and home management skills
How to get support
You will need to discuss with your current support worker to make a referral for you. They will help you plan for a move to a new support worker.
