Find out about our services that provide short to medium term accommodation for people who require a supported environment before moving on to their own home.
Who we help
We support people:
- to manage their mental health, regain functional living skills and build a supportive network in their move towards independent living
- who are homeless or at risk of homeless
- transitioning from prison where there is a mental health and housing need
- transitioning from a higher level of supported living (step down in care and support)
What we do
We work in partnership with Leeds and York Partnership Foundation Trust (LYPFT) and external organisations such as housing agencies, to support people out of hospital and back into the community.
Our three mental health accommodation services provide the following support:
- a prevention service with LYPFT to support individuals for up to 2 weeks to avoid a hospital admission or to avoid a delayed discharge from hospital
- strength-based support – we help people achieve the things they want to do in their everyday lives
- signposting to other groups and organisations
- working with carers, family and friends to offer support, information and advice
- one-to-one and group activities and support
