Change of address
If you move, you need to
let us know. Then we can make sure there is no gap in your care and support.
Leaving Leeds
If you move to an area which falls under another council, we will work with your new council to make sure there's no break in your care.
Before you move, your new council will assess your needs and agree a new support plan with you. If this is not possible the new council will arrange the care and support we outlined in your previous plan.
Changes in your needs or finances
If your needs or finances change, get in touch with your social worker and let them know.
If you do not have a social worker or you do not know their contact details, you can
contact us instead. We'll arrange for your social worker or another member of their team to call you back.
To discuss the finances of someone you look after, you will need the legal right to speak for them. Find out
how to represent someone.
Reviewing your needs
We will have a conversation with you to find out what has changed, and work out the best way to support you.
This means there might be changes to the
types of care and support you get and
how much you pay for it.
We’ll send you a copy of your new care and support plan.
If you want to change someone's care because you're worried they are being treated badly, we can take action to make sure they're safe.
